The Extended Schools Activity Proposal (ESAP) Form
The ESAP (Extended Schools Activity Proposal) form has been in use for several years. It has been transferred from an electronic version to a Word document in response to difficulties experienced by schools. The replacement form asks all the same questions as the on-line version, is linked to the same advice and guidance but is easier to use. The form can be partly completed and printed off for sharing with colleagues before being fully completed and submitted. A completed version can be printed off for filing as a record of the original proposal.
It is hoped that headteachers and governors will find this form clearer to read and more practical to use.
The person completing the form will be asked to confirm that the relevant guidance has been read by ticking a declaration on the last page. Forms that are incomplete will be returned to be completed in full before being considered by the Extended Schools Panel.

Completion of the form will be necessary for most additional school activities but the following paragraphs give more detail about completing the form. You will need to fill in this form when:
The Governing Body is considering developing extended services i.e. a service that is being provided by or on behalf of the school and is in addition to the ‘normal’ school day. Schools may wish to consider a range of activities that can be delivered including childcare, after school clubs, adult learning provision and multi agency working, all of which can provide pupils and families in their community with a variety of opportunities.
The completion of this form will help schools address the following questions:
- What type of activity do we need to provide?
- Who will be held accountable for the provision?
- What structure will we use to monitor provision?
- With whom do we need to consult?
- How will the activity be funded?
- Who will employ the staff?
- Will the provision be located on the school site or elsewhere – have we considered leasing arrangements and health and safety?
Governing Bodies should be mindful that they should always consider viability and sustainability when deciding on any provision to be offered.
The completion of this form will also ensure that the governing body has complied with the requirement to consult with the local authority before establishing an extended service.
This form need not be completed for those traditional activities that enhance the life of the school and pupils within it e.g. the extra-curricular activiites such as the football team or the rehearsal for a school production.
